To be effective, health information must be structured and codified, so that there is a single interpretation, regardless of the place and time in which it was produced and will be consumed. The modeling of this information is a crucial point in achieving this purpose, as it systematizes the use of methods and standards in the development of information models, with the acquiescence of health actors committed to the process. The present study presents a complete health information modeling process, appropriate to the Brazilian context as it adopts the standards of the National Health Data Network (RNDS). The process is in the application and validation phase, in the context of cooperation between a Federal University and a State Health Secretariat, responsible for a health care network.
