Objective: To develop a system for managing care for patients with wounds in primary and secondary care settings within the Brazilian Unified Health System and to evaluate its usability and applicability. Methods: A methodological study was conducted in three stages: requirements definition, system development, testing and usability evaluation in a simulated setting, and usability evaluation by nurses in a controlled clinical setting. Data were analyzed using descriptive statistics, and usability was assessed using the System Usability Scale. Results: The system automatically generates 4 clinical, 16 quality-of-care, and 5 financial indicators. The overall usability score in the simulated setting was 73. In clinical practice, the score was 79.4 in primary care and 76.9 in secondary care. The system was found to be applicable after use in 143 patient encounters. Conclusion: The system is innovative and generates clinical, quality-of-care, and financial indicators; it demonstrated applicability and excellent usability in the settings studied.
